Assumption of Risk & Release of Liability
I understand that participation in camp activities may involve inherent risks, including but not limited to slips, falls, accidents, allergic reactions, and the use of art materials and equipment. On behalf of myself and my child, I voluntarily assume all risks associated with participation in Crazy Picasso / Tipsy Canvas camp activities.
To the fullest extent permitted by law, I release, waive, and hold harmless Crazy Picasso, Tipsy Canvas, their owners, employees, contractors, volunteers, and agents from any claims, demands, injuries, damages, losses, or liabilities arising out of or related to my child’s participation in camp activities, except where caused by gross negligence or willful misconduct. I further agree to be financially responsible for any damage to studio property caused by my child beyond ordinary wear and tear.
Medical Authorization
In the event of illness, injury, or emergency, I authorize Crazy Picasso / Tipsy Canvas and its representatives to obtain emergency medical treatment for my child if I cannot be reached. I understand that Crazy Picasso / Tipsy Canvas does not provide medical insurance coverage and that all medical expenses incurred on behalf of my child are my responsibility.
Health Disclosure
I certify that my child is physically able to participate in camp activities and that I have disclosed any medical conditions, allergies, medications, behavioral concerns, or special accommodations that may affect my child’s participation or safety.
Photo & Media Release
I grant permission for Crazy Picasso / Tipsy Canvas to photograph, videotape, or otherwise record my child during camp activities and to use such images, recordings, artwork, or likenesses for promotional, educational, advertising, social media, website, and other business purposes without compensation. I understand that these materials may be used in print or digital formats and may remain in use indefinitely.
Camp Policies
I understand and agree that children may bring a drink but no food due to allergy and safety concerns. Any person picking up my child must be listed as an authorized pickup person. Children must be signed in and signed out by a parent or authorized guardian. Late pickup fees are $10 for the first five minutes and $1 per minute thereafter.
Parent/Guardian Acknowledgment
I certify that I am the parent or legal guardian of the child being registered and have the authority to enter into this agreement on the child’s behalf. I have carefully read this Parent Consent, Liability Waiver, Medical Authorization, and Photo Release, understand its contents, and voluntarily agree to all terms and conditions. By submitting this form, I acknowledge that my electronic signature has the same legal effect as a handwritten signature.
